Clinical Features

Brachydactyly, a bone dysostosis condition, is characterized by shortening of fingers and/or toes. Temtamy preaxial brachydactyly syndrome (also called preaxial brachydactyly syndrome, Temtamy type) is a congenital syndrome characterized by mental retardation, sensorineural deafness, growth retardation, and bilateral symmetric preaxial brachydactyly and hyperphalangism. Some patients may also have dental anomalies and cleft palate (Temtamy and Aglan 2008).

Genetics

CHSY1-related Temtamy preaxial brachydactyly syndrome is inherited in an autosomal recessive manner. The CHSY1 protein is a secreted FRINGE enzyme that interacts with the NOTCH signal pathway (Tian et al. 2010). About 10 unique pathogenic variants have been reported: missense (4), nonsense (2) splicing (1) and small deletion (3). No large deletions/duplications have been reported (Human Gene Mutation Database; Tian et al. 2010).

Clinical Sensitivity - Sequencing with CNV PGxome

Due to limited publications, clinical sensitivity is difficult to predict. Analytical sensitivity should be high, because all reported pathogenic variants are missense, nonsense, splicing, and small ins/del (Human Gene Mutation Database; Li et al. 2010; Tian et al. 2010).

Testing Strategy

This test provides full coverage of all coding exons of the CHSY1 gene plus 10 bases of flanking noncoding DNA in all available transcripts along with other non-coding regions in which pathogenic variants have been identified at PreventionGenetics or reported elsewhere. We define full coverage as >20X NGS reads or Sanger sequencing. PGnome panels typically provide slightly increased coverage over the PGxome equivalent. PGnome sequencing panels have the added benefit of additional analysis and reporting of deep intronic regions (where applicable).

Dependent on the sequencing backbone selected for this testing, discounted reflex testing to any other similar backbone-based test is available (i.e., PGxome panel to whole PGxome; PGnome panel to whole PGnome).

Indications for Test

Candidates for this test are patients with symptoms consistent with Temtamy preaxial brachydactyly syndrome and the family members of patients who have known CHSY1 pathogenic variants. This test may also be considered for the reproductive partners of individuals who carry pathogenic variants in CHSY1.
